Weight Management Service

Use this service to check your eligibility for a referral to an NHS weight management service.

The weight management services support adults living with obesity who also have a diagnosis of diabetes, high blood pressure (hypertension) or both, to manage their weight and improve their health.

Have you had a diagnosis of any of these conditions?
Have you had a diagnosis of type 2 diabetes within the last 6 years?
Have you had a blood sugar (HbA1c) measurement taken within the last 12 months?
Do you take diabetes medication?
Was the blood sugar measurement between 43 to 87 mmol/mol?
Was the blood sugar measurement between 48 to 87 mmol/mol?
Did you have a diabetes review when it was last offered (including eye screening)?
Will you continue to have annual reviews even if you are in remission?
